Specialised dermatology services

Specialised dermatology services are services, which may not be provided in every dermatology department in local general hospitals. The services include the investigation and treatment of rare diseases and the management of severe diseases not suitable for, or not responding to conventional treatment available in most dermatology departments.

Phototherapy at St. John’s Department of Dermatology

The phototherapy unit offers a wide range of ultraviolet light treatments and testing. Ultraviolet B, both narrow band UVB and broad band UVB, is widely used in treating psoriasis and other disorders. PUVA (psoralen + UVA) is available for both whole body and local (hand and foot) treatments. PUVA is used in treating psoriasis, cutaneous T cell lymphoma, vitiligo, graft vs host disease and many other disorders. Light testing is also performed to help in the diagnosis of sun sensitivity.

Dermatosurgery and cosmetology

The Division of Dermatologic Surgery offers a comprehensive range of diagnostic and therapeutic services for the management of diseases or disfigurement afflicting skin, hair and nails. Procedures such as skin and nail biopsy, cryotherapy, electrocautery, radiofrequency, acne scar revision surgeries, cellular and tissue graft surgeries for vitiligo ,nail surgeries, ear lobe repair , excision of birth marks and cysts excision, Platelet Rich Plasma (PRP) therapy for hair loss and wound healing are performed by specially trained dermatosurgeons.

The department is equipped with state of the art LASER machines to provide cosmetic treatments such as permanent hair reduction, acne scar corrections, skin resurfacing, removal of vascular and pigmented lesions and also tattoos.

The department also offers skin rejuvenation and anti-ageing procedures such as chemical peels, microdermabrasion, microneedling, Botox and filler injections.

The one year full time fellowship program in Dermatosurgery offered by the department, recognized by the Rajiv Gandhi University, is one of only three such programmes in the state.

Severe inflammatory skin disease requiring non-conventional therapy.

Management of difficult cases of commoner diseases, for which second or third line treatments or supportive care are not normally available in a most hospitals often require a multi-disciplinary approach to management. Examples include: Severe psoriasis; Severe eczema; Immunobullous disease; Auto immune disease; Severe drug reactions, Severe/complex connective tissue disease, etc.

Management of Life threatening skin disease

A multidisciplinary approach is used in the management of life threatening skin disorders such as Toxic epidermal necrolysis, staphylococcal scalded skin syndrome, generalised pustular psoriasis, Immunobullous disease including severe pemphigus and other severe drug reactions as they often require interfaces with burns unit, intensive therapy units, microbiology and plastic surgery in their management.

Photo-investigation and specialised photo-dermatology

Includes the investigation and management of porphyrias and light induced skin disease, such as chronic actinic dermatosis, photo allergy, light sensitive eczema, and actinic prurigo.

Sexually Transmitted Infections

Difficult cases and unusual disorders may require management by a specialised multidisciplinary team. This service links in to specialised cancer services, HIV/AIDS treatment and care, and specialised services for infectious diseases.

HIV and infectious diseases of the skin

HIV/AIDS diagnosis, treatment and care services in dermatology provides linkages to the PPTCT, VCTC and ART centre at the institution. Infectious diseases requiring specialised dermatological care include some complex fungal and viral infections, cutaneous tuberculosis, adverse drug reactions amongst others.

Leprosy

Diagnosis and management of not only the routine cases but also complicated cases. The management issues in these situations often require a multidisciplinary approach involving various departments such as orthopedics, PMR, Ophthalmology,…etc.

Some services are provided within centres of dermatology, although they may remain

linked to pathology departments.
