The above Fellowship Programme/Certificate course is affiliated to St. John’s National Academy of Health Sciences, and the course completion certificate will be issued by St. John’s Medical College, Bengaluru.

INSTRUCTIONS

• Attach copies of relevant certificates.
• Attach a detailed biodata.
• Include proof of payment (UTR/transaction number).
• The subject of the email should be the name of the fellowship applied for.
